Knox-Keene Grievance/Complaint Process

For California Residents Only

If you are a California resident and you have a grievance or complaint with us, please call us at 858-736-0037, send us an email to bryan.levy@allonehealth.com or file your grievance or complaint with us by mail to 5405 Morehouse Dr, Suite 200, San Diego, CA 92121. We will work hard to resolve your grievance or complaint within 30 days of receipt.

Please note that if there is an immediate threat to your health or your request was denied as experimental/investigational, you may seek immediate assistance from the California Department of Managed Health Care (“DMHC”) by calling 888-466-2219 or follow this link to file your grievance/complaint with DMHC. In other circumstances, you are required to file a grievance or complaint regarding each issue/request with us and participate in the process for 30 days before you submit a complaint to the DMHC.
